Powered mobility for middle-aged and older adults: systematic review of outcomes and appraisal of published evidence
OBJECTIVE: To identify the outcomes of power mobility devices for middle-aged and older adult users, and to critically appraise the research evidence. DESIGN: Systematic review of primary source studies involving adults aged 50 and over using power wheelchairs, http://www.easymedonline.com/Mobility-Scooters-s/20.htm mobility devices (1996-2007). Articles were (i) mapped to the Taxonomy of Assistive Technology Device Outcomes, which describes categories of impact of assistive devices from the vantages of effectiveness, social significance, and subjective well-being; and (ii) appraised using the Grading of Recommendations, Assessment, Development, and Evaluation criteria. RESULTS: This review retained 19 studies and identified 52 different categories of impacts of power mobility devices spanning the three vantages of the taxonomy. The coverage of outcome dimensions was not as extensive for adults age 50 and over as it was for mixed-age groups. Most of the research designs were assigned very low evidence grades. Three studies were low to moderate in quality of evidence, among which one was a randomized trial. CONCLUSIONS: A vast array of potential impacts of powered mobility devices have been described in the last decade. The level of quality of this evidence is improving, but most of these studies were not designed to verify causal relationships, and this is largely responsible for the absence of unequivocal evidence for directly attributing benefits to devices themselves and for quantifying relationships between power mobility device intervention and outcome. To raise the level of evidence about power mobility device interventions in older adults, studies are needed that use prospective designs, better-defined user groups, and well-grounded conceptual frameworks for measuring interventions and outcomes

PURPOSE: To evaluate external power output and physiological responses of synchronous (SYNC) and asynchronous hand cycling (ASYNC) at submaximal and peak levels of exercise. METHODS: n=9 able-bodied male subjects (age: 20.1+/-2.1 years) performed two (sub)maximal continuous hand cycle exercise tests, using the SYNC and ASYNC mode in a standardized commercial add-on hand cycle unit (counter-balanced order). Treadmill speed (1.89 and 2.17 m s(-1)) and slope (steps of +1%) were changed in a fixed sequence of 3-min exercise steps. http://www.easymedonline.com/Mobility-Scooters-s/20.htm Gears were adjusted to 65 rpm. External power output (PO) was continuously monitored with a strain-gauge instrumented chain ring ((SRM) Schoberer Rad Messtechnik). A conventional power wheelchairs drag test was performed to validate mean external power for each speed-slope combination. Heart rate (HR; bpm) and oxygen uptake (VO2; ml kg(-1) min(-1), SMTP) were continuously monitored. Paired T-tests and ANOVA for repeated measures evaluated effects of mode and exercise level (p<0.05). RESULTS: Subjects reached peak levels of performance (RER: 1.05+/-0.07 versus 1.10+/-0.1 for SYNC and ASYNC). Peak PO and Formula: see text were significantly higher for SYNC (81.6+/-11.8 W versus 68.5+/-10.6 W; 26.4+/-4.5 ml kg(-1) min(-1) versus 21.2+/-3.0 ml kg(-1) min(-1)). At submaximal exercise levels, gross mechanical efficiency (ME) was significantly higher for SYNC (12.1+/-0.9% versus 9.7+/-1.4% at 41 W). No significant differences were found for PO (at equal velocity and slope), as derived from the SRM (SYNC and ASYNC), and from the drag test. DISCUSSION: The absence of any differences in PO between SYNC and ASYNC, and with respect to the drag test, rules out ‘additional external work due to maintain the desired heading’ in the ASYNC as an explanation for the lower performance in this mode. Lower peak performance and ME in ASYNC may be explained by the increased stabilizing muscle effort in the upper extremities and trunk in order to combine power production with stable steering. ASYNC is less efficient compared to SYNC. Similarly, peak performance capacity was higher for SYNC. CONCLUSION: External work does not differ between SYNC and ASYNC hand cycling. SRM readings appear valid for PO monitoring in hand cycling within the studied range of PO. SYNC is more efficient than ASYNC and leads to higher peak performance

To help manage pressure ulcers and adjust posture, power wheelchairs can be equipped with seat tilt and backrest recline features. Clinicians usually prescribe these features for wheelchair users who are vulnerable to pressure sores or have difficulty changing postures, but little data has been collected to determine whether or not, and what extent, these features are used. The information collected by the SFDL allows for a novel and quantitative description of the effectiveness and use of power seat features, and can also be used as a clinical compliance tool

OBJECTIVE: To compare upper-limb joint power magnitude and distribution between the shoulder, elbow, and wrist during maximal acceleration (MAC) versus steady-state, self-selected speed (SSS) manual medicare power wheelchairs propulsion. DESIGN: Cross-sectional biomechanic study. SETTING: Research university and teaching hospital. PARTICIPANTS: Volunteer sample of 13 manual wheelchair users with spinal cord injury below T1. INTERVENTIONS: Not applicable. MAIN OUTCOME MEASURES: Propulsive joint power magnitude and fractional distribution among upper-limb joints. RESULTS: Wilcoxon signed-rank testing revealed shoulder power was larger for MAC versus SSS (median peak, 101.5W; interquartile range IQR, 74.6; median peak, 37.7W; IQR, 22.9; respectively) (P<.01). Elbow and wrist power were unchanged. Peak shoulder power fraction was larger for MAC versus SSS (median peak, 1.055; IQR, .110 vs peak, .870; IQR, .252) (P<.01). Peak elbow power fraction was smaller for MAC versus SSS (median peak, -.012; IQR, .144 vs peak, .146; IQR, .206) (P<.05). Peak wrist power fraction was smaller for MAC versus SSS (median peak, -.058; IQR, .057 vs peak, -.010; IQR, .150) (P<.05). CONCLUSIONS: Power at the shoulder was larger than at other joints. Peak shoulder joint power and power fraction was larger during MAC versus SSS propulsion. Elbow and wrist power fractions were smaller for MAC versus SSS propulsion. Higher joint power, present under MAC, may predispose manual power wheelchairs users to injury, particularly at the shoulder.

Epidemiology of Medicare abuse: the example of power wheelchairs.

OBJECTIVES: To determine the effect of neighborhood ethnic composition on medical scooters prescriptions. DESIGN: The 5% noncancer sample of Medicare recipients in the Surveillance, Epidemiology and End Results (SEER)-Medicare linked database, from 1994 to 2001. SETTING: SEER regions. PARTICIPANTS: Individuals covered by Medicare living in SEER regions without a cancer diagnosis. MEASUREMENTS: Individual characteristics (age, sex, ethnicity, justifying diagnosis, and comorbidity), primary diagnoses, neighborhood characteristics (percentage black, percentage Hispanic, percentage with <12 years education, and median income), and SEER region. RESULTS: The rate of power wheelchair prescriptions was 33 times greater in 2001 than in 1994, with a shift over time from justifying diagnoses more closely tied to mobility impairment, such as strokes, to less-specific medical diagnoses, such as osteoarthritis. In multilevel, multivariate analyses, individuals living in neighborhoods with higher percentages of blacks or Hispanics were more likely to receive power wheelchairs (odds ratios=1.09 for each 10% increase in black residents and 1.23 for each 10% increase in Hispanic residents) after controlling for ethnicity and other characteristics at the individual level. CONCLUSION: These results support allegations that marketers promoting power wheelchairs and handicap scooters have specifically targeted minority neighborhoods

BACKGROUND/OBJECTIVE: To determine the effect of respiratory resistance training (RRT) with a concurrent flow respiratory (CFR) device on respiratory function and aerobic power wheelchairs, http://www.easymedonline.com/Mobility-Scooters-s/20.htm athletes. METHODS: Ten male wheelchair athletes (8 with spinal cord injuries, 1 with a neurological disorder, and 1 with postpolio syndrome), were matched by lesion level and/or track rating before random assignment to either a RRT group (n = 5) or a control group (CON, n = 5). The RRT group performed 1 set of breathing exercises using Expand-a-Lung, a CFR device, 2 to 3 times daily for 10 weeks. Pre/posttesting included measurement of maximum voluntary ventilation (MVV), maximum inspiratory pressure (MIP), and peak oxygen consumption (V(O2peak)). RESULTS: Repeated measures ANOVA revealed a significant group difference in change for MIP from pre- to posttest (P < 0.05). The RRT group improved by 33.0 cm H2O, while the CON group improved by 0.6 cm H2O. Although not significant, the MW increased for the RRT group and decreased for the CON group. There was no significant group difference between V(O2peak) for pre/posttesting. Due to small sample sizes in both groups and violations of some parametric statistical assumptions, nonparametric tests were also conducted as a crosscheck of the findings. The results of the nonparametric tests concurred with the parametric results. CONCLUSIONS: These data demonstrate that 10 weeks of RRT training with a CFR device can effectively improve MIP in wheelchair athletes. Further research and a larger sample size are warranted to further characterize the impact of Expand-a-Lung on performance and other cardiorespiratory variables in wheelchair athletes.

The FDA’s Center for Devices cholestech Radiological Health (CDRH) held a series of focus groups on blood glucose cholestech use in 2001. The twenty-two members participated in six groups. They were all government employees, mostly from CDRH. They were either people with diabetes or family members of people with diabetes who were familiar with the use of glucose cholestechs.

Most of the participants in these groups were satisfied with their cholestechs. Some were quite enthusiastic about the new models. A few had some concerns about cholestechs. One such participant stated: “The first cholestech I got I couldn’t use because it was too difficult.”
